Know Your Appointment Type

Annual Physical vs. Medicare Annual Wellness Visit

These appointments are often confused, but they are not the same. Knowing which visit you need helps our team schedule the correct appointment and explain what may be included.

Annual Physical

Preventive physical examination

An annual physical generally includes a preventive health review and physical examination based on your age, medical history and current health needs.

May include:

  • Review of medical and family history
  • Review of current medications and allergies
  • Vital signs and selected physical examination
  • Preventive screening recommendations
  • Discussion of nutrition, activity and health risks
  • Laboratory testing when ordered by the physician
Coverage note: Insurance benefits for annual physicals vary. Some plans cover one preventive physical each year, while others may apply cost-sharing or exclude certain services.

Medicare Annual Wellness Visit

Medicare preventive planning visit

A Medicare Annual Wellness Visit focuses on preventive planning, health risks and updating your personalized prevention plan. It is not the same as a complete head-to-toe physical examination.

May include:

  • Health-risk assessment
  • Review of medical history, medications and care team
  • Routine measurements such as blood pressure and weight
  • Cognitive and safety screening questions
  • Review of preventive screenings and vaccines
  • Personalized prevention and follow-up plan
Medicare note: The Medicare Annual Wellness Visit generally does not include routine laboratory testing or a comprehensive physical examination unless separately ordered or performed.
New medical concerns may be billed separately. When new symptoms, chronic conditions, medication changes or other problem-focused concerns are evaluated during a preventive visit, your insurance may process an additional office-visit service with applicable copay, coinsurance or deductible.

Frequently Asked Questions

Wellness Visit Questions

Find answers to common questions about preventive physicals, Medicare wellness visits, screenings, medications and insurance coverage.

What ages does Harbor Physicians see for wellness visits?

Harbor Physicians provides preventive care, physicals and wellness visits for patients ages 14 and older.

How often should I schedule a wellness visit?

Many patients schedule preventive care once each year. The appropriate timing depends on your age, health history, insurance benefits and your physician’s recommendations.

Is a wellness visit the same as an annual physical?

Not always. A routine annual physical may include a preventive examination, while a Medicare Annual Wellness Visit focuses on health risks, preventive planning and updating your personalized prevention plan.

Does a Medicare Annual Wellness Visit include a physical exam?

A Medicare Annual Wellness Visit is not generally a complete head-to-toe physical examination. It typically focuses on health history, risk assessment, routine measurements and preventive planning.

Are laboratory tests included in my wellness visit?

Laboratory testing is not automatically included in every preventive visit. Your physician may order testing based on your age, health history, risk factors and medical needs. Insurance coverage may vary.

Can I discuss new symptoms during my physical?

Yes, but evaluating new symptoms, changing medications or managing an existing condition may result in an additional problem-focused service. Your insurance may apply a copay, coinsurance or deductible to that portion of the visit.

What should I bring to my wellness appointment?

Bring your photo identification, current insurance card, relevant medical records and all current medication bottles whenever possible. This includes prescriptions, over-the-counter products, vitamins and supplements. A current pharmacy-generated medication list is acceptable when bottles cannot be brought.

Why should I bring my medication bottles?

Medication bottles allow your physician to verify the exact name, strength, dosage, directions and prescribing provider. This helps reduce medication errors, duplicate therapies and conflicting information.

Will my insurance cover a wellness visit?

Preventive benefits vary by insurance plan. Some plans cover annual preventive visits with limited or no patient cost, while certain tests, vaccines or problem-focused services may be billed separately. Please contact your insurer or call our office for help identifying the correct appointment type.

Can Harbor Physicians help coordinate recommended screenings?

Yes. Your physician may recommend appropriate laboratory testing, imaging, cancer screenings or specialist referrals based on your age, history and individual risk factors.

Do you offer school, work or sports physicals?

Availability depends on the requested form, examination requirements and appointment type. Please call (904) 368-6809 before scheduling and let our team know which form or clearance is required.
