Understanding Medication Coverage
Many weight-loss and diabetes medications require prior authorization before an insurance company will consider coverage. The process begins only after the prescription has been processed by your pharmacy.
Provider Sends the Prescription
If the physician determines that medication is medically appropriate, the prescription is transmitted to your selected pharmacy.
Pharmacy Processes the Prescription
The pharmacy submits the claim to your insurance company to determine whether the medication is covered or requires prior authorization.
Pharmacy Sends the PA Request
If authorization is required, the pharmacy sends the request to Harbor Physicians electronically or by fax. We must receive this request before we can begin.
Harbor Submits the Information
Our office completes the requested form and submits available medical history, diagnoses, laboratory results and treatment documentation.
Insurance Reviews the Request
The insurance company reviews the request and may approve it, deny it or request additional records. Processing time varies by plan.
Decision and Pharmacy Follow-Up
After a decision is issued, the pharmacy can process the medication if approved or discuss the rejection and available next steps.

Does Harbor Physicians prescribe GLP-1 medications?
GLP-1 and related medications may be considered as part of an individualized medical weight-management plan. Eligibility depends on medical history, current treatment, BMI, health conditions, potential risks, laboratory findings and the provider’s clinical judgment.
Can I request Wegovy or Zepbound specifically?
You may discuss any medication with the physician, but requesting a specific product does not guarantee it will be prescribed. The provider may recommend a different medication, a non-medication approach, additional testing or specialist evaluation.
Do I need diabetes to receive weight-loss treatment?
Not necessarily. Medical weight-management options depend on the reason for treatment, the medication being considered, your health history and applicable prescribing criteria. Insurance coverage rules may be different from medical prescribing criteria.
Is there a required BMI for prescription treatment?

Will my insurance cover weight-loss medication?
Coverage varies widely. Some plans exclude medications prescribed for weight management, while others require specific diagnoses, BMI criteria, laboratory results, previous treatment attempts or step therapy. Harbor Physicians cannot guarantee coverage or approval.
When can Harbor Physicians begin my prior authorization?
The provider first sends the prescription to your pharmacy. The pharmacy processes it through your insurance and sends a prior authorization request to Harbor Physicians when one is required. Our office cannot begin until that request is received.
How long does prior authorization take?
Processing time varies by insurance plan. Delays may occur when the insurer requests additional records, requires step therapy, processes an appeal or has not yet received all required information. The insurance company controls the final review timeline.
What happens if insurance denies the medication?
The office can review the denial reason and discuss possible next steps. Depending on the circumstances, options may include submitting additional records, considering an appeal, selecting a covered alternative or discussing other medical weight-management treatments.
Can Orange Park Pharmacy help if insurance denies coverage?
When appropriate, the prescription may be sent to Orange Park Pharmacy. The pharmacy may be able to help identify available manufacturer programs, coupons or lower cash-pricing options. Savings and medication availability are not guaranteed.
How often will I need follow-up appointments?
Follow-up frequency depends on the treatment plan, medication, response, side effects, laboratory results and clinical needs. Regular appointments are required to monitor safety, progress and whether treatment should continue or be adjusted.
How quickly should I expect to lose weight?
Results vary considerably. Medical weight management focuses on safe, sustainable progress rather than an overnight result. Response depends on treatment, health conditions, medication tolerance, nutrition, activity and consistency with follow-up.
What if the medication causes side effects?
Contact Harbor Physicians for instructions. Do not increase, decrease, stop or restart medication on your own. Seek urgent or emergency care for severe symptoms such as major difficulty breathing, loss of consciousness, severe allergic reactions or other life-threatening concerns.
Can I use medication prescribed to another person?
No. Prescription weight-loss medications require individualized medical evaluation and monitoring. Never share medication, use another person’s prescription or obtain prescription treatment from an unverified source.
Do you treat patients younger than 18?
Harbor Physicians provides primary care for patients ages 14 and older. Weight-management evaluation for a teen depends on age, development, health history, guardian participation, clinical need and whether referral to an appropriate pediatric or specialty provider is recommended.
