What should I bring to my appointment?
Bring your photo identification, current insurance card, relevant medical records and the physical bottles for every prescription, over-the-counter medication, vitamin and supplement you currently take. If you cannot bring the bottles, bring a current printed medication list directly from your pharmacy.
Why are the physical medication bottles important?
The physical bottles allow your physician to verify the exact medication name, strength, dosage, directions and prescribing provider. This helps reduce medication errors, duplicate therapies and conflicting dosage information.
Do you provide preventive physicals and wellness visits?
