Primary Care
Dr. Akhtar brings more than 40 years of medical experience to Harbor Physicians. His approach combines attentive listening, comprehensive evaluation and personalized treatment to support each patient's physical and emotional well-being.
Internal Medicine
Dr. Omer grew up in Orange Park and completed his internal medicine residency at Orange Park Hospital. His hospital and primary care experience allows him to evaluate a wide range of conditions while providing thoughtful, patient-centered care.

Dr. Osman grew up in Orange Park and completed his internal medicine residency in Florida. He provides careful evaluation, clear communication and individualized treatment for routine and complex medical needs.

Patients ages 14 and older are welcome. Harbor Physicians provides preventive care, chronic condition management, medication management and specialty coordination from our Orange Park office.
Harbor Physicians participates with many Medicare, Medicaid, Marketplace and commercial insurance plans. Participation may vary by product and provider network.
Participation varies by the exact product. MyBlue HMO is not accepted. Some other Blue plans may also be out of network. Please call with your insurance card so we can verify your specific plan.
Dual Complete HMO is not accepted. Dual Complete PPO may be accepted depending on the specific network and member plan.

We provide primary care, annual physicals, Medicare wellness visits, chronic condition management, preventive care, medical weight management, thyroid evaluation, men’s health services, women’s preventive care, in-office testing, minor procedures, referrals and care coordination. Services are based on medical need and physician evaluation.
